AST Palliative Care Case Based Discussion (CBD)

Case Based Discussion enrolment

Ensure that you have read the CBD section of the Fellowship Assessment Handbook for guidance on Case requirements.

To complete your enrolment, you will be required to submit:

• At least 4 compliant cases* including a Case Cover Page (noting 12 Cases are required within 12 months to complete this assessment)
• A Cases Summary form
• A Community Profile - for each community where cases are sourced from

* NB: Each Case is saved as a single PDF file of 5mb or less

By enrolling into this assessment, I am confirming the following:

• Cases include clinical notes with a minimum of one (1) and maximum of four (4) consultations for each case with clear demonstration that I am primarily managing the patient.
• Case page limit does not exceed 20 - 30 pages per case.
• Cases include a range of cases from across the AST curriculum.
• Cases are de-identified as per the CBD requirements.
• Permission from my employer to submit the cases has been obtained and is recorded on the Cases Summary Form.
