AST Palliative Care Case Based Discussion (CBD)

Case Based Discussion enrolment

Ensure that you have read the CBD section of the Fellowship Assessment Handbook for guidance on Case requirements.

To complete your enrolment, you will be required to submit:

• At least 4 compliant Cases including a Case Cover Page (noting 12 Cases are required within 12 months to complete the CBD)
• A Cases Summary form
• A Community Profile – for each community where cases are sourced from

Prior to uploading your documents check the following:

• Cases include a range of cases from across the AST curriculum
• Cases are de-identified, including the removal of patient and family names, phone numbers, addresses, Medicare, radiology and hospital numbers
• You have permission from your employer to submit the cases (recorded on the Cases Summary form)
• Each Case includes a Case Cover Page, consultation notes, relevant investigations and/or evidence of follow ups, referral to specialist or other health care providers
• Each Case is saved as a single PDF file of 5mb or less
