National Prediabetes and Diabetes Mellitus Audit - 01/01/2026 - 19/12/2026

Diabetes is the fastest growing chronic disease in Australia and affects approximately 5% of the population,with Aboriginal and Torres Strait Islander Australians almost four times more likely than non-Indigenous Australians to have type 2 diabetes.It is believed that around 30% of Australians with diabetes are undiagnosed.

This newly revamped audit is unique in the education realm as it not only focuses on identifying prediabetes but also provides an alert system for clinicians. This system will assist clinicians and give them the tools to assist in the identification and recall of your patients with prediabetes and assists in the monitoring of patients already diagnosed with diabetes type 1 or 2. Only accessible via our online secure Medway portal audit reports and all patients follow up reports who are identified will be available 3 monthly. The audit will occur each calendar available JAN- DEC.

Learning objectives

-Recognise patients with impaired blood glucose levels that are higher than normal.

-Develop and implement a multipronged, multidisciplinary care plan tailored to each prediabetic patient.

-Review and identify processes related to patient follow-up and abnormal diagnostic results to ensure nothing is overlooked.

-Demonstrate and educate patients on appropriate glycaemic targets for individuals with type 1 and type 2 diabetes, considering factors such as age and comorbidities. 

-Create systematic approaches to help patients with type 1 and type 2 diabetes achieve their glycaemic targets, individualizing their HbA1c targets.

-Clinicians can review their current practice standard, against best practice guidelines to achieve better support and care for patients.

 

Assessment information

Following registration of the audit, access will be given into our secure Medway site where clinicians will be sent an email alert when reports and patient follow up report is delivered, this will occur 3 monthly throughout the calendar year. Clinicians will have access to the list of patients identified with prediabetes and those managing their Diabetes Mellitus who may require more attention to their condition Name DOB Address MC number so that follow up can occur. 

An added advantage with our online portal clinicians,  will be able to visualise graphically their patients path through prediabetes and Diabetes Mellitus. 

Evaluations will be sent 6 and 12 monthly where clinicians can report and reflect of findings.

Additional information

Statistically working from references, MC data and interviews with GPs, the average full time GP will manage in their practice approx 30 patients (slightly lower than average) with Diabetes Mellitus either type 1 or 2.

Identified patients who may prediabetic working on the above references etc at a min  have 5 over the 12 month period.

Above will be the expected criteria for achievement for the audit. Clinicians full time over the 12 month audit cycle will be expected to manage 30 patients living with Diabetes Mellitus and will identify 5 patients in their practise with prediabetes
