This course focuses on suturing and perfecting the ellipse relating to skin cancer medicine. Participants will learn about the most common type of excision (elliptical excision). The detailed marking out process consists of dotting and assessing the edge of lesion, determining solid line surgical margins and designing the ellipse. The unit is illustrated with mathematical diagrams and clinical images of performing the ellipse on various parts of the anatomy. Unit two describes how best to cut the ellipse and how to hold the scalpel. Diagrams and images demonstrate the best practice approach to cutting the ellipse. Videos of blade handle tilt and cutting ends of the ellipse are illustrated. Unit three lists different types of sutures and describes the pros and cons of each suture type. These include dermal, the slider, dermal pulley, butterfly dermal, simple interrupted, mattress and continuous sutures. Knotting techniques for sutures are described.

This course focuses on local anaesthesia relating to skin cancer medicine. Participants will learn about the pharmacology and guidelines of the two main classes of local anaesthetic - amides (lignocaine, bupivacaine, prilocaine) and esters (procaine, benzocaine, cocaine). The process of infiltration is listed, including bevel and needle orientation. Practical tips for the patient are outlined to minimise wound damage due to site numbness. The second part of this course focusses on injecting the ear and lip, paying attention to sensory nerve blocks including the trigeminal nerve, infra-orbital nerve, mental nerve and temporal nerves. ‘Tiger country’ complexities are described and supported by diagrams and images. Areas of extreme complexity like Erb's point, facial nerve (temporal branch and marginal branch), and the common peroneal nerve are outlined. Signs and symptoms of nerve injury are included and practice tasks are provided.

This course focuses on biopsies, haemostasis and halo grafting relating to skin cancer medicine. Participants will learn about monopolar and bipolar modes of cautery, including pros and cons of each mode. Electrical and fire risks of cautery are mentioned. The unit concludes with haemostasis options for punch and shave biopsy. Procedures and sizes of punch biopsies are given in addition to the use of marini sutures for biopsy closure if required. The second unit looks at shave biopsies and shave excisions. Attention is paid to using shave excisions for excising melanoma. The technique explains the advantages and limitations of halo grafting, and lists the three skin graft phases - plasmatic imbibition, Inosculation and neovascularization. Clinical diagrams, mathematical equations and recordings are provided. The final section of this unit outlines suitable dressings and dressing changes. Recordings of procedures on pigskin and in the clinical setting are provided.

This course focuses on curettage and cautery relating to skin cancer medicine. Participants will learn about the comparison of thermal injury with liquid nitrogen to mechanical and thermal injury with electrical desiccation. A table of (pre-)malignant conditions, the freeze time, freeze-thaw cycle and halo diameter are included. The process of cryotherapy (cryosurgery) is explained including the appropriate use of cryosurgery including which lesions and locations. The use of a cryogun is described and illustrated by several videos. The second part of this course focusses on when to use curettage and cautery and appropriate anatomical locations for curettage, divided into three zones of recurrence - high, middle and low risk. The process of curettage and cautery is listed and supported by videos and images. Tips for diagnosis of small basal cell carcinomas including a patient case of a small BCC on the nose finalize this course.

This course focuses on ellipse planning relating to skin cancer medicine. Participants will learn about the three priorities in skin cancer surgery: cut the cancer out, close the resulting defect including the limitations of the ellipse, and minimising scar lines. The unit reviews the design of the ellipse and applies the ellipse around different anatomical locations with consideration to skin tension lines. Different techniques are demonstrated to determine the best direction to design an ellipse. Diagrams are included of skin tension lines on the face, limbs and chest. Images and videos illustrate the learning in this unit. The second unit focusses on ellipse planning on various areas of the face and hands. Curving ellipses may be used when following skin tension lines. Clinical recordings and images are provided. ‘Tiger country’ is reinforced at the conclusion of this course.

This course focuses on the big three – BCC, SCC and melanoma relating to skin cancer medicine. Participants will learn about the clinical and dermoscopic features of BCCs, biopsy options, pathology and treatment approaches. The course then moves to solar keratosis and examines the use of topical non-surgical treatments. Unit two continues with field-directed treatment methods including photodynamic therapy, and lists the pros and cons of available treatments. Two patient cases are included. Unit three covers SCC lesions and outlines the clinical and dermoscopic features, biopsy options, pathology and treatment approaches. Clinical cases including images are provided for various SCCs. Unit four focusses on melanomas and sentinel lymph nodes and again outlines the clinical and dermoscopic features, biopsy options, pathology and treatment approaches. Clinical cases are provided for various melanomas.

This course focuses on preparation for surgical treatment relating to skin cancer medicine. Participants will learn the fundamental principles when preparing for and performing skin cancer surgery. The fundamentals include an aseptic approach using sterile equipment, sutures and dressings and emphasizes the importance of clean skin, operator preparation and environment. The unit then looks at local anaesthetics and haemostasis. Equipment for wound closure and sutures are listed including the fundamentals of closing the defect. Recommended suture removal times and the suture techniques, use of prophylactic antibiotics and anticoagulants conclude this unit. Unit two describes the punch and shave biopsy and includes a video and clinical images of these procedures.

This course focuses on excision and repair relating to skin cancer medicine. Participants will learn the instruments used for wound closure and sutures used for skin closure, including suture technique and removal times. Drawings and description of the simple interrupted suture are discussed, including a recording of this procedure. Unit two emphasizes the importance of using an absorbable suture and shows drawings of a conventional buried dermal suture. The unit concludes with a recording of this procedure. Unit three focuses on the preparation for an elliptical excision, including placement of excisions for the best cosmetic result. It outlines the steps leading to performing the ellipse including surgical marking up. The procedure is described, and illustrated by drawings of the incision considering maximal skin tension lines and fusiform excision lines. The unit concludes with a video of this procedure.

Participants will learn about the progression of solar keratosis into invasive squamous cell carcinoma and the role of immunosuppression, illustrated by clinical examples. Field treatment options including modes of action and clinical examples of progression of treatment for each modality are provided. Side effects and a table of expected local skin reactions is provided. The complexities when treating immunosuppressed patients are considered. The course concludes with a description of photodynamic therapy, including clinical examples.

This course focuses on factors influencing the choice of treatment in skin cancer medicine. The course looks at treatment options including surgical excision and considers tumour characteristics and the patient and treatment modalities. The course then looks at management options and guidelines for superficial/pigmented/non-pigmented BCC. Surgical excision is not always the best treatment option and these situations are discussed and management plans outlined. The course then looks at hedgehog pathway inhibitors for the treatment/prevention of advanced BCC. The management of keratinocyte skin cancer including clinical and dermoscopic grading and treatment options for solar keratosis are included. Invasive SCC including prognostic classification and characteristics/differentiation of keratocanthoma are included. The course concludes with lentigo maligna, one of the most difficult to recognise melanoma types.
