care, support chronic disease management, and work closely with patients,
families and multidisciplinary teams to achieve positive health outcomes.
The role

contribute to population health initiatives, quality improvement activities,
service development and health equity outcomes for Aboriginal and Torres Strait
Islander peoples. You will also support supervision and teaching activities,
